New General Data Protection Regulations (GDPR) 2018
As you may be aware new General Data Protection Regulations (GDPR) come into force on the 25th May 2018, being transparent and providing accessible information to patients about how we will use your personal information is a key element in the Data Protection Act 2018 and EU General Data Protection Regulations. How We Use Your Information
We collect and hold data about you for the purpose of providing safe and effective healthcare and we will only share your information with your consent.
- If you are happy with how we use your information you do not need to do anything
- If you do not want us to use your data beyond providing your care please let us know and we will update your records accordingly
- If you do not give consent for us to share your data with other health care providers your treatment options may be limited, we will inform you if this is the case

Medical Records Request
In accordance with the the Data Protection Act 2018 and the EU General Data Protection Regulations (GDPR) you have the right to request access to your records, you can do this by completing the subject access request form opposite and bringing it into the practice.
